Labeling of IngredientscriticalInspector note: The grab and go items do not have the complete label. Provide a complete ingredient label or keep the items behind the counter so the employee can answer questions about allergens and ingredients. (Items moved to behind the counter for today) 4/23/19 At the onset of the inspection the grab and go sandwiches and salads are still in the grab and go refrigerator with incomplete labels. Items that are packaged and in a grab and go customer self-service area need a complete label that includes all ingredients including sub-ingredients and major allergens.

PIC KnowledgecriticalInspector note: The manager does not have her certified food manager certificate or allergen certificate. Provide a certified food manager and allergen trained manager. 4/23/19 There is no evidence that the onsite manager has signed up for a certified food manager course or allergen awareness course. The certificates provided are for managers at the other two locations.
