Inspector note: PIC should be monitoring and supervising cleaning and extermination schedules. Waltham made last visit on 6-12-17 and has not returned per the weekly schedule. Rodent droppings were observed in the same places in the establishment that I observed on the initial inspection. Several small flies were still observed around hand sink next to 3 bay sink and in the back prep area. PIC is to provide a daily cleaning and sanitizing checklist to be done every day and initialed by person. All floors are to be inspected for rodent droppings daily and are to be removed and floors are to be cleaned daily. Walls are to be cleaned daily of spills and spatter.
